Please click on the link to open the specified form. After the form is open, please fill out the requested information, click the "print" button, print the form and return to school August 5, 2009. When all required forms have been filled out and returned to the school, you will receive your carline number. Notaries will be on-hand the day of orientation to notarize your forms.

FBCS utilizes the FACTS Tuition Management Program. If you choose the payment plan to pay your tuition, you will need to enroll in the FACTS program via a link on our website. If you choose to pay in full, you do not need to enroll with FACTS. We are only offering the 1 pay or 11 pay options. Please click on the link above for more information about FACTS.
To set up your FACTS tuition agreement, please go to the FACTS online enrollment site log in to your enrollment account.
